Requirements for dynaMACS on the Cloud:
 Cloud fees are billed on a quarterly basis
 Cloud fees are in addition to the normal dynaCARE or subscription fees
 dynaMACS on the Cloud will not operate if the cloud subscription or dynaCARE fees are not paid.
 Cloud fees can only be cancelled with a written 6 month notice.
